We will need the following information:

(If you want to remove any words or numbers please state that in the notes)
(If you want the shirt to be the colors in the picture please state that in the notes)

- First name for hood (one name only)
- Last name for back (one name only)
- Your child's number (one number only)
- Your team colors (Two colors only)
